Authorization to Investigate

You grant our payment providers the right to investigate your credit, residence, and employment histories, including requesting your consumer credit reports. This authorization applies both to evaluate your application and to service any account you may establish, remaining valid throughout the life of your account.

You authorize CareFund to share your information with one or more third-party payment providers in connection with your interest in a transaction. By doing so, you provide written instructions to these providers, under the Fair Credit Reporting Act, to access your credit profile or other information from consumer reporting agencies such as Equifax, Experian, or TransUnion. This information will be used solely to conduct a pre-qualification, which will not impact your credit score unless specified otherwise.

Consent to Communications

You agree that CareFund and US Bank may contact you via phone, text, or email for transactional purposes. Communications may include prerecorded or artificial voice messages and automated dialing devices. Your wireless service provider may charge you for these communications.

For application and collection purposes, US Bank may contact your employer, landlord, or other third parties to verify your employment, residence, or location as allowed by law.

You will be required to sign a credit agreement with one or more payment providers, agreeing to the terms offered.

If approved by US Bank:

• The terms of your account, including the Annual Percentage Rate and repayment terms, may differ from those initially provided with your application.
• Insurance options will not be available even if requested during your application process.

Consumer Consent to Electronic Communication (E-Sign)

You consent to receiving communications from CareFund electronically, including legal disclosures, notices, and policy updates, unless you withdraw this consent as described below.

Electronic Communications Include:

• Legal and regulatory disclosures related to your account or products/services.
• Notices about changes in terms or responses to claims.
• Privacy policies and marketing-related emails.

Methods of Delivery:

• Via email.
• By accessing a designated website link provided in an email notice.
• By downloading a PDF file containing the communication.

• A computer with internet access.
• A current web browser with 128-bit encryption (e.g., Firefox 72.x, Safari 5.x, Chrome 79+).
• A valid email address, which can be updated by emailing info@carefundmarket.com.
• Adequate storage or a printer for saving communications.
• Add @everly.market to your safe senders list if you use a spam filter.

Withdrawing consent may terminate your CareFund account. Withdrawal will be effective after we process your request.

By consenting, you agree that electronic communications hold the same legal weight as paper communications. We recommend you print or save copies of important disclosures.

AML and eCBSV Compliance

As part of our Anti-Money Laundering (AML) program, CareFund and its payment providers may verify your identity using the electronic Consent-Based Social Security Number Verification (eCBSV) system and other data sources. This is to ensure compliance with federal regulations and protect against fraudulent activity.

Credit Inquiry Consent

By submitting your application, you authorize a soft inquiry on your credit profile to evaluate eligibility. If you proceed with an offer, you consent to a hard inquiry for account booking purposes, which may impact your credit score.
